Auricular reconstruction can be challenging when an acquired defect requires both structural support and soft tissue coverage. A 2-stage repair is often required. This includes contralateral ear cartilage harvest and a local flap to provide vascular wound coverage. A later stage is required for flap division. A large wedge resection is an alternative single-stage option, but this results in a dramatically smaller ear. A flip-flop flap is another option in this location, but this can alter the ear projection.
