Efficacy and safety of intravenous sotalol for termination of paroxysmal supraventricular tachycardia

A double-blind, placebo-controlled, crossover, multicenter study was conducted to study the efficacy and safety of a single intravenous dose of sotalol (1.5 mg/kg over 10 minutes) in achieving normal sinus rhythm in paroxysmal supraventricular tachycardia (SVT) lasting greater-than-or-equal-to 15 minutes. Patients were randomized to either sotalol or placebo as initial treatment, and if the SVT was not terminated a crossover was performed after 20 minutes. A total of 43 patients were enrolled, 38 of whom with spontaneous (n = 14) or induced (n = 24) SVT were analyzed for sotalol efficacy. Most patients (n = 27) had atrioventricular (AV) nodal reentrant tachycardia, and an important subgroup (n = 11) had circus movement tachycardia, using an accessory pathway for retrograde conduction. The number of patients converting to sinus rhythm as a result of the initial treatment was significantly higher in the sotalol group than in the placebo group, for spontaneous (p < 0.005) as well as for induced tachycardia (p < 0.001). Sinus rhythm was achieved within 30 minutes in 83% of all patients who received sotalol as the first drug, compared with 16% of the patients first receiving placebo (p < 0.001). For sotalol safety analysis, 42 patients were included. A total of 37 patients received sotalol, 19 as the first treatment, and 18 as the second treatment, while 25 patients received placebo. A total of 15 possible adverse effects were reported, occurring in 10 patients with sotalol versus 4 with placebo. The only severe side effect (hypotension) necessitating termination of drug administration occurred with placebo. No proarrhythmic effects were observed. The intravenous administration of sotalol appears to be safe and effective for acute termination of supraventricular reentrant tachycardia.

Implantable cardioverter defibrillator: data storage and retrieval with Patientlog

A pacemaker management system (Patientlog), implemented on a IBM personal computer (AT), was adapted for the control and the administrative management of patients with an implantable cardioverter defibrillator (ICD). Several parameters used for pacemakers were also suitable for ICDs, while some fields were defined within the available frame of the database, to cope with the specific technical information of ICDs. "Intervention-cards" display diagnostic (threshold) as well as complex surgical information (approach, lead systems, etc.). "Follow-up-cards" present charge time and the number of shocks in such a way that decision making is very easy. Fourteen patients were followed with a total of 21 interventions, six different types of devices, and more than 150 follow-ups. A correct control of ICD functions is easy. The system is adapted for programmable ICDs. Updated reports containing necessary information are generated immediately after each procedure or follow-up.

Thermal comfort and indoor air quality on end-user satisfaction level evaluation in a Nearly Zero Carbon neighbourhood

End-user satisfaction studies in residential buildings has to be approached combining the user's perspective and technical criteria to consider the complex interactions influencing the building energy performance. Therefore, in this study the physical characteristics of dwellings and their environments are assessed, user satisfaction is examined, and the relationship between them is investigated. The study aims to illustrate the end-user satisfaction in exemplary high performance buildings and to investigate how the users are interacting with these buildings. Examination of the building performance, thermal comfort and indoor air quality are the main focal points of the work. In general, results reflect a significant improvement on the satisfaction level of the inhabitants with the comfort of the dwelling after the refurbishment of the district. Findings from the cross-analysis of both surveys and measurements are used to further refine conclusions and identify the driving factors of the interrelationship between building performance and end-user satisfaction.

The Augment in Homer, with special attention to speech introductions and conclusions

In this article, we will show that the use of the augment in Homeric speech introductions and conclusions was not random, ut could be explained by morphometric, syntactic and semantic constraints. Later, these rules were no longer understood: the augment became a mandatory marker of past tense in Greek prose, while its absence in Homer (which was also motivated by rules and constraints) was reinterpreted as an archaism and an element of the poetic language. The article only focuses on Homeric speech introductions and conclusions, and leaves out a discussion of Homer and epic poetry in general, of Mycenaean and the other Indo-European languages that have the augment.

Des loups de mer couverts de tatouages, de solides dockers, des armoires à glace derrière leur volant ; la cliché qui voudrait que l'industrie du transport ne soit qu'une affaire de costauds est désormais dépassé. Leur vie n'en reste pas moins une suite de moments forts, de tâches harassantes, toujours dangereuses et rarement bien payées. Pourtant, au cours de ces cent derrières années, peu de choses ont changé autant et aussi rapidement que le monde du transport: les bateaux, les ports, les routes, les voitures... Et les travailleurs ont été constraints de s'adapter. Les photos les plus anciennes sont particulièrement marquantes: de pauvres hères tirant un bateau depuis le chemin de halage, une femme de batelier au gouvernail avec son enfant à ses côtés au bout d'une corde, des dockers ployant sous des sacs de céréales de septante kilos ou davantage. Le contraste avec les clichés d'aujourd'hui est énorme. Cet ouvrage relate d'une façon très visuelle la vie de ces travailleurs, de leurs épouses, de leurs enfants et de leurs amis au sein du monde de transport qui s'est radicalement transformé entre 1913 et 2013.

Stang's Law and the Indo-European word for 'cow'

The present article investigates the etymology of the Indo-European word for "cow" and looks at two types of reconstruction, with and without laryngeal: *g(w)ous and *g(w)eh(3)us (suggested by Kurylowicz in 1927) or *g(w)h(3)eus (as already suggested by de Saussure in 1878). By assessing the instances where Stang's Law operated and failed to operate, we find that the correct reconstruction is *g(w)ous. The accusative singular of the word "cow" in Doric and Homeric Greek, Sanskrit, Sabellic and the Greek epsilon x alpha tau o mu beta eta are additional evidence in favour of this reconstruction. The article also looks at two possible arguments against that reconstruction (the short vowel a in the oblique cases and the disyllabic scansion of the first syllable in Vedic poetry): an ablaut type *nokwts, *nek(w)ts can account for the short a in the Indo-Iranian weak cases, and the absence of a disyllabic scansion in Avestan proves that the Vedic metre is the fruit of a poetic licence. Consequently, we see no reason why *gwous could not be kept.*

Netherlandish stained glass roundels in private collections in nineteenth-century Ghent: display, function and meaning

In the beginning of the nineteenth century, the Southern Netherlands saw the rise of a renewed interest in national artistic and antiquarian heritage among local amateurs. Perhaps rather unexpectedly, the city of Ghent was praised as a centre of private collecting, equal to the traditional artistic capitals Brussels and Antwerp. The local press as well as foreign (mostly German and English) travellers praised the Flemish town for its numerous rich collections of "national antiquities". The local collectors had a particular preference for Netherlandish stained glass from the fifteenth to the seventeenth centuries, due to the decorative, artistic and national-historical value these objects had for them. Based on contemporary publications such as travel literature, articles published in periodicals and preserved collection and auction catalogues, this paper will trace the various meanings stained glass adopted for the Ghent private collectors. Not only did the integration of the panels into the decorative scheme of the collectors' cabinets allow for the creation of historicizing interiors that functioned as an appropriate setting for the display of other art objects and antiques. Stained glass roundels were moreover considered true national monuments, since it was believed that the art of painting on glass had been brought to the highest degree of perfection in the Southern Netherlands. Furthermore, I aim to relate the emerging interest in stained glass among Ghent collectors to the Gothic revival and the associated collecting of this art in Britain. On the one hand, the increasing demand for historical stained glass on the British market raised awareness of the precarious situation of these "national monuments" in their country of origin. On the other hand, however, the British neo-Gothic movement also provided aesthetic models for continental collectors.

What Role Does Allergy Play in Chronic Ear Disease and Laryngitis?

Abstract

IgE-mediated allergy plays a well-established role in both nasal and pulmonary diseases due to the common epithelium and shared mediator responses of the upper and lower airways. This "unified airway" concept has also been described in other sites within the head and neck that contain similar respiratory mucosa: the middle ear and the larynx. This review will highlight the data suggesting a role for IgE-mediated allergic disease in chronic laryngopharyngeal and middle ear disease and the role for allergy testing to aid in diagnosis and treatment of these disorders.

A Swelling over Sternum in a Child: Reminder of an Uncommon Diagnosis

Lack of awareness about isolated tubercular osteomyelitis of the sternum resulted in a delay in diagnosing the condition in an eleven-year-old girl who presented with a gradually increasing swelling over the sternum. Radiological, histological, and microbiological investigations helped diagnose the condition and the child responded well to antitubercular therapy (ATT) and surgical debridement. The report provides a brief description about the various management options available.

Ibrutinib combined with bendamustine and rituximab compared with placebo, bendamustine, and rituximab for previously treated chronic lymphocytic leukaemia or small lymphocytic lymphoma (HELIOS): a randomised, double-blind, phase 3 study

Background: Most patients with chronic lymphocytic leukaemia or small lymphocytic lymphoma relapse after initial therapy. Bendamustine plus rituximab is often used in the relapsed or refractory setting. We assessed the efficacy and safety of adding ibrutinib, an oral covalent inhibitor of Bruton's tyrosine kinase (BTK), to bendamustine plus rituximab in patients with previously treated chronic lymphocytic leukaemia or small lymphocytic lymphoma. Methods: The HELIOS trial was an international, double-blind, placebo-controlled, phase 3 study in adult patients (>= 18 years of age) who had active chronic lymphocytic leukaemia or small lymphocytic lymphoma with measurable lymph node disease (>1.5 cm) by CT scan, and had relapsed or refractory disease following one or more previous lines of systemic therapy consisting of at least two cycles of a chemotherapy-containing regimen, an Eastern Cooperative Oncology Group (ECOG) performance status of 0-1, and adequate bone marrow, liver, and kidney function. Patients with del(17p) were excluded because of known poor response to bendamustine plus rituximab. Patients who had received previous treatment with ibrutinib or other BTK inhibitors, refractory disease or relapse within 24 months with a previous bendamustine-containing regimen, or haemopoietic stem-cell transplant were also excluded. Patients were randomly assigned (1:1) by a web-based system to receive bendamustine plus rituximab given in cycles of 4 weeks' duration (bendamustine: 70 mg/m.intravenously on days 2-3 in cycle 1, and days 1-2 in cycles 2-6; rituximab: 375 mg/m.on day 1 of cycle 1, and 500 mg/m.on day 1 of cycles 2-6 for a maximum of six cycles) with either ibrutinib (420 mg daily orally) or placebo until disease progression or unacceptable toxicity. Patients were stratified according to whether they were refractory to purine analogues and by number of previous lines of therapy. The primary endpoint was independent review committee (IRC)-assessed progression-free survival. Crossover to ibrutinib was permitted for patients in the placebo group with IRC-confirmed disease progression. Analysis was by intention-to-treat and is continuing for further long-term follow-up. The trial is registered with ClinicalTrials.gov, number NCT01611090. Findings: Between Sept 19, 2012, and Jan 21, 2014, 578 eligible patients were randomly assigned to ibrutinib or placebo in combination with bendamustine plus rituximab (289 in each group). The primary endpoint was met at the preplanned interim analysis (March 10, 2015). At a median follow-up of 17 months (IQR 13.7-20.7), progression-free survival was significantly improved in the ibrutinib group compared with the placebo group (not reached in the ibrutinib group (95% CI not evaluable) vs 13.3 months (11.3-13.9) in the placebo group (hazard ratio [HR] 0.203, 95% CI 0.150-0.276; p<0.0001). IRC-assessed progression-free survival at 18 months was 79% (95% CI 73-83) in the ibrutinib group and 24% (18-31) in the placebo group (HR 0.203, 95% CI 0.150-0.276; p<0.0001). The most frequent all-grade adverse events were neutropenia and nausea. 222 (77%) of 287 patients in the ibrutinib group and 212 (74%) of 287 patients in the placebo group reported grade 3-4 events; the most common grade 3-4 adverse events in both groups were neutropenia (154 [54%] in the ibrutinib group vs 145 [51%] in the placebo group) and thrombocytopenia (43 [15%] in each group). A safety profile similar to that previously reported with ibrutinib and bendamustine plus rituximab individually was noted. Interpretation: In patients eligible for bendamustine plus rituximab, the addition of ibrutinib to this regimen results in significant improvements in outcome with no new safety signals identified from the combination and a manageable safety profile.

Another attempt at a chronology for Grassmann’s Law in Greek

This article takes another look at the chronology of Grassmann's Law (GL). It investigates the different dating suggestions - Indo-European, pre-Mycenaean, post-Mycenaean, post-Homeric - and assesses them critically. The article first holds that the evidence from Germanic, Latin and Iranian indicates that GL did not operate in PIE and secondly agrees with Ruijgh, Lejeune, Janko, Hajnal, Plath and Vine that GL only operated after the Mycenaean era. To prove this, the article uses the evidence from Mycenaean, Homeric scansion, and augmentation and reduplication in the Greek verbal morphology. The article finally finds that there are some indications to put GL after the period in which the Homeric poems were written down (somewhere between the end of the 9th - beginning 8th century BC in our opinion).*

What is the Greek counterpart of (Proto-)Indo-Iranian (*)th?

Almost since the beginning of Indo-European linguistics as a science, it has been noted that the voiceless aspirates (Tenues Aspiratae, henceforth TA) were relatively infrequent and were only attested in very few languages (Indo- Iranian, Greek and Armenian). Usually, Indic and Greek agreed in having a TA, but in a number of instances a Sanskrit or Indo-Iranian aspirate corresponded to a plain plosive in Greek. In most of these instances a Sanskrit th corresponded to a Greek t, but there were instances where Sanskrit th was matched by a Greek th. The article therefore focuses mainly (but not exclusively) on the instances in which a th can be found in Indo-Iranian and discusses what the Greek counterpart is. First, we state the problem and give a brief overview of previous suggestions to solve this problem, but none of these explanations can explain the differences and agreements between the Greek and Indo-Iranian. After the overvie we proceed to an analysis of all the instances and argue that the difference in aspiration between Greek t and (Proto-)Indo- Iranian *th can be explained by the fact that the (Proto-)Indo-Iranian forms are the result of a cluster *th2V or are due to evolutions, specific to Indo-Iranian, Indic or Iranian. Where the Greek counterpart of (Proto-)Indo-Iranian *th is th, we argue that the aspirate has to be posited for PIE or — in case there is no related word in a Western language is lacking — for East-IE.

The etymology of Greek sigma theta epsilon nu omicron sigma

The present article investigates the different etymologies that have been suggested for Greek sigma theta epsilon nu omicron sigma. It starts by analysing the meaning and the instances in the oldest literary texts. It then confronts the suggested etymologies with the Mycenaean evidence and proper names in Thracian and Celtic. The article finds that the two most commonly accepted etymologies are problematic because they require the presence of labiovelars but that presence is not corroborated by the Mycenaean evidence. Proper names in Thracian and Celtic also seem to question the accepted etymologies, but these names are not entirely clear. As a result of the Mycenaean evidence, another etymology has to be suggested.

Ruwe zeebonken, stoere dokwerkers, geharde truckers: het cliché wil dat werken in de transportindustrie enkel voor de 'zware mannen' is weggelegd. Hun levens zijn voer voor sterke verhalen, altijd gevaarlijk, en zelden goed betaald. Toch veranderde de afgelopen 100 jaar niets zo snel en zo zichtbaar als het transport: de schepen, de havens, de wegen, de auto's. En de werkmens, die paste zich aan. De vroege foto's tarten alle verbeelding: een arm gezin trekt een boot vanop een jaagpad langs de kanalen, een schippersvrouw houdt haar kind aan boord aan de leiband, in de haven sleuren mannen met graanzakken van zeventig kilo of meer. Het contrast met de foto's van nu is immens. Dit boek vertelt op een bijzonder beeldende wijze het levensverhaal van de arbeiders, hun vrouw, kinderen en vrienden, in de spectaculair snel veranderende wereld van het transport, van 1913 tot 2013.

Evidence for laryngeal aspiration in Greek? Part I: The 'recent' evidence

Since de Saussure, H. Pedersen and Kurylowicz the Sanskrit (and Indo-Iranian) voiceless aspirates are considered to be the result from the combination of a plain voiceless plosive and a laryngeal (*h(2) according to most scholars), and, consequently, the existence of phonemic voiceless aspirates in PIE is no longer accepted. In more recent times, an evolution similar to that of Indo-Iranian has been assumed for Greek as well but the Greek situation is less convincing and has not been investigated as thoroughly as the Indo-Iranian branch. This article investigates the "recent" evidence in favour of the laryngeal aspiration in Greek (Peters 1993a and b, Werba 1996, Mayrhofer 2005, Matzinger 2005 and Vine 2006) and finds that most examples are not conclusive: either the reconstruction is not conclusive or there is an example under exactly the same circumstances that argues against aspiration. The older evidence (used by Sturtevant, Collinge, Hamp, Klingenschmitt and Isebaert) will be discussed in part 2.*

Randomized, double-blind, phase III trial of enzastaurin versus placebo in patients achieving remission after first-line therapy for high-risk diffuse large B-cell lymphoma

Purpose: To compare disease-free survival (DFS) after maintenance therapy with the selective protein kinase C (PKC) inhibitor, enzastaurin, versus placebo in patients with diffuse large B-cell lymphoma (DLBCL) in complete remission and with a high risk of relapse after first-line therapy. Patients and Methods: This multicenter, phase III, randomized, double-blind, placebo-controlled trial enrolled patients who were at high risk of recurrence after rituximab-cyclophosphamide, doxorubicin, vincristine, and prednisone (R-CHOP). Patients (N = 758) with stage II bulky or stage III to IV DLBCL, three or more International Prognostic Index risk factors at diagnosis, and a complete response or unconfirmed complete response after 6 to 8 cycles of R-CHOP were assigned 2:1 to receive oral enzastaurin 500 mg daily or placebo for 3 years or until disease progression or unacceptable toxicity. Primary end point was DFS 3 years after the last patient entered treatment. Correlative analyses of biomarkers, including cell of origin by immunohistochemistry and PKC expression, with efficacy outcomes were exploratory objectives. Results: After a median follow-up of 48 months, DFS hazard ratio for enzastaurin versus placebo was 0.92 (95% CI, 0.689 to 1.216; two-sided log-rank P = .541; 4-year DFS, 70% v 71%, respectively). Independent of treatment, no significant associations were observed between PKC protein expression or cell of origin and DFS or overall survival. Conclusion: Enzastaurin did not significantly improve DFS in patients with high-risk DLBCL after achieving complete response to R-CHOP. Achievement of a complete response may have abrogated the prognostic significance of cell of origin by immunohistochemistry.
