Nutrition and lifestyle in European adolescents: the HELENA (Healthy Lifestyle in Europe by Nutrition in Adolescence) study
Adolescence is a critical period, because major physical and psychologic changes occur during a very short period of time. Changes in dietary habits may induce different types of nutritional disorders and are likely to track into adulthood. The aim of this review is to describe the key findings related to nutritional status in European adolescents participating in the HELENA (Healthy Lifestyle in Europe by Nutrition in Adolescence) study. We performed a cross-sectional study in 3528 (1845 females) adolescents aged 12.5-17.5 y. Birth weight was negatively associated with abdominal fat mass in adolescents and serum leptin concentrations (in female adolescents), providing additional evidence for a programming effect of birth weight on energy homeostasis control. Breakfast consumption was associated with lower body fat content and healthier cardiovascular profile. Adolescents eat half of the recommended amount of fruit and vegetables and less than two-thirds of the recommended amount of milk and milk products but consume more meat and meat products, fats, and sweets than recommended. For beverage consumption, sugar-sweetened beverages, sweetened milk, low-fat milk, and fruit juice provided the highest amount of energy. Although the intakes of saturated fatty acids (FAs) and salt were high, the intake of polyunsaturated FAs was low. Adolescents spent, on average, 9 h/d of their waking time (66-71% and 70-73% of the registered time in boys and girls, respectively) in sedentary activities. Factors associated with adolescents' sedentary behavior included the following: 1) age; 2) media availability in the bedroom; 3) sleeping time; 4) breakfast consumption; and 5) season. Sedentary time was also associated with cardiovascular risk factors and bone mineral content. In European adolescents, deficient concentrations were identified for plasma folate (15%), vitamin D (15%), pyridoxal 5'-phosphate (5%), beta-carotene (25%), and vitamin E (5%). Scientists and public health authorities should raise awareness of the importance of a healthy and sustainable lifestyle as a foundation of the health of the European population, now and in the future.

Abatacept experience in steroid and rituximab-resistant focal segmental glomerulosclerosis
Primary focal segmental glomerular sclerosis (FSGS), one of the major causes of nephrotic syndrome, eventually results in end-stage renal disease. Currently, FSGS is treated with immunosuppressive therapies, which include calcinuerin inhibitors (cyclosporine), glucocorticoids, B-cell depleting agents (rituximab) and, recently, a T-cell co-stimulatory inhibitor (abatacept). Until recently, there had been no cases reporting resistance to all current therapies. We report a case of a 62-year-old Caucasian man with biopsy-proven FSGS, who responded well to oral prednisolone therapy. However, 2 years later, he had a relapse and failed to respond to prednisolone. Subsequent treatments then included cyclosporine, rituximab and cyclophosphamide, which were not successful. The patient was then administered abatacept, a novel T-cell co-stimulatory inhibitor—though he did not experience any side effects, there was no change in proteinuria nor in creatinine.

Extramedullary plasmacytoma of the gingiva
Description
Extraosseous plasmacytoma, also referred to as extramedullary plasmacytoma (EMP), is defined by ICD-10 as a localised plasma cell neoplasm that arises in tissues other than bone.1 It is considered one of the three variants of plasma cell neoplasms, the other two being multiple myeloma (MM) and solitary bone plasmacytoma (SBP) (also known as medullary plasmacytoma).
EMP is a relatively rare lesion, constituting 3% of all plasma cell neoplasms.2 About 1% of head and neck tumours are EMPs. It is found most commonly in the head and neck region, with 80% of cases occurring in the nasopharynx, paranasal sinuses and tonsils.3 EMPs occur less commonly in the gingiva. The first case was documented by Martinelli and Rulli4 in 1968 as a sessile neoplasm in the gingiva from the lower left to right canine, which could be confused with chronic gingivitis....

Management of a severe case of Gitelman syndrome with poor response to standard treatment
Gitelman syndrome is an autosomal recessive distal renal tubular disorder caused by defective sodium chloride transporters. Biochemically, it presents with hypokalaemic metabolic alkalosis, hypomagnesaemia and hypocalciuria. It is usually managed with oral potassium supplements and potassium-sparing diuretics. We report a case of a 28-year-old woman whose condition worsened during pregnancy; she became resistant to standard management after delivery of her second child. She was managed in a specialist metabolic clinic through a comprehensive approach including perseverance with oral potassium supplement, weekly intravenous potassium and magnesium infusion, correction of vitamin D level and the offering of appropriate dietary advice; this controlled the patient's symptoms and prevented repeated hospital admissions. In this case report, we illustrate a patient's presentation and diagnosis with Gitelman syndrome, discuss triggers of exacerbation, review the relevant literature in terms of differential diagnoses and provide practical advice on the management of difficult cases in a specialist clinic.

Enquiring About Tolerance (EAT) study: Feasibility of an early allergenic food introduction regimen
Publication date: Available online 17 February 2016
Source:Journal of Allergy and Clinical Immunology
Author(s): Michael R. Perkin, Kirsty Logan, Tom Marrs, Suzana Radulovic, Joanna Craven, Carsten Flohr, Gideon Lack
BackgroundThe influence of early exposure to allergenic foods on the subsequent development of food allergy remains uncertain.ObjectiveWe sought to determine the feasibility of the early introduction of multiple allergenic foods to exclusively breast-fed infants from 3 months of age and the effect on breastfeeding performance.MethodsWe performed a randomized controlled trial. The early introduction group (EIG) continued breastfeeding with sequential introduction of 6 allergenic foods: cow's milk, peanut, hard-boiled hen's egg, sesame, whitefish (cod), and wheat; the standard introduction group followed the UK infant feeding recommendations of exclusive breastfeeding for around 6 months with no introduction of allergenic foods before 6 months of age.ResultsOne thousand three hundred three infants were enrolled. By 5 months of age, the median frequency of consumption of all 6 foods was 2 to 3 times per week for every food in the EIG and no consumption for every food in the standard introduction group (P < .001 for every comparison). By 6 months of age, nonintroduction of the allergenic foods in the EIG was less than 5% for each of the 6 foods. Achievement of the stringent per-protocol consumption target for the EIG proved more difficult (42% of evaluable EIG participants). Breastfeeding rates in both groups significantly exceeded UK government data for equivalent mothers (P < .001 at 6 and at 9 months of age).ConclusionEarly introduction, before 6 months of age, of at least some amount of multiple allergenic foods appears achievable and did not affect breastfeeding. This has important implications for the evaluation of food allergy prevention strategies.

Point triangulation through polyhedron collapse using the l∞ norm
Multi-camera triangulation of feature points based on a minimisation of the overall l2 reprojection error can get stuck in suboptimal local minima or require slow global optimisation. For this reason, researchers have proposed optimising the l∞ norm of the l2 single view reprojection errors, which avoids the problem of local minima entirely. In this paper we present a novel method for l∞ triangulation that minimizes the l∞ norm of the l∞ reprojection errors: this apparently small difference leads to a much faster but equally accurate solution which is related to the MLE under the assumption of uniform noise. The proposed method adopts a new optimisation strategy based on solving simple quadratic equations. This stands in contrast with the fastest existing methods, which solve a sequence of more complex auxiliary Linear Programming or Second Order Cone Problems. The proposed algorithm performs well: for triangulation, it achieves the same accuracy as existing techniques while executing faster and being straightforward to implement.

Fast and robust variational optical flow for high-resolution images using SLIC superpixels
We illustrate how a pixel-based method can be directly applied on a near-regular grid of segments (superpixels) from a well-chosen image segmentation. This allows the accelerated processing of high-resolution content with existing pixel-based methods, which might not be able to process it in its input resolution in the first place. In essence, the superpixel segmentation is a tool to decrease the dimensionality of the input image without significant loss of quality for the final result. We show how optical flow estimation, whose execution time is often of vital importance, can benefit from this workflow for processing 4K content whilst also conferring a degree of noise robustness: this pre-processing is a novel direction of research for the field. Naturally, the application of this concept is not restricted to optical flow: it is applicable in many different fields.

Acute ataxie: een zeldzame diagnose?
The medical history of a three-year-old boy with acute ataxia due to the opsoclonus-myoclonusataxia syndrome (short: opsoclonus-myoclonus syndrome (OMS)) is presented. The further diagnostic investigation reveals an associated thoracic neuroblastoma. In children with acute ataxia, the diagnosis of OMS is rare, difficult and often unrecognized in the beginning. In half of the children with OMS, an associated neuroblastoma is present. Further investigation for an associated neuroblastoma is always indicated when the diagnosis of OMS is set.

Accurate modeling of the predicted kESM-based link performance metric for BIC-OFDM systems
Effective SNR mapping (ESM) is a powerful technique for optimizing the performance of orthogonal frequency division multiplexing (OFDM) based wireless systems. ESM transforms a vector of subcarrier SNRs into a scalar effective SNR, which represents the SNR that would yield the same error performance in an equivalent system operating over an additive white Gaussian noise (AWGN) channel. This technique significantly simplifies the development of adaptive coding and modulation (ACM) algorithms. As the distribution of the effective SNR, when only imperfect and outdated channel state information (CSI) is available, cannot be expressed in closed form, we develop an approximate statistical model which is based on the beta distribution. Our model is compared in terms of approximation accuracy against models based on the Gaussian distribution, the gamma distribution and the more complex Pearson and generalized extreme value (GEV) distributions.

BPMN extensions for decentralized execution and monitoring of business processes
Software-as-a-service (SaaS) providers are further expanding their offering by growing into the space of business process outsourcing (BPO). Therefore, the SaaS provider wants to administer and manage the business process steps according to a service level agreement. Outsourcing of business processes results in decentralized business workflows. However, current business process modeling languages, e.g. Business Process Execution Language (BPEL), Business Process Model and Notation (BPMN), are based highly on a centralized execution model and current BPMN engines offer limited constructs for federation and decentralized execution. To guarantee execution of business processes according to a service level agreement, different parties involved in a federated workflow must be able to inspect the state of external workflows. This requires advanced inspection interfaces and monitoring facilities. Current business process modeling languages must thus be extended to support monitoring in the s pecification, support modeling and support deployment of decentralized workflows. In this paper, correlation and monitoring extensions for BPMN are described. These extensions to BPMN are described such that the existing specification can still be used as is in a backwards compatible way.

Omega-3 fatty acids as an adjunct for periodontal therapy—a review
Abstract
Objectives
The aim of this article is to present an overview of omega-3 fatty acids, their anti-inflammatory properties and potential use as an adjunct for periodontal therapy.
Materials and methods
A general literature search was conducted to provide an overview of omega-3 fatty acids, their metabolism and anti-inflammatory properties. A more specific literature search of PubMed and EMBASE was conducted to identify articles dealing studies investigating the effects of omega-3 fatty acids in the treatment of periodontitis in animals and humans and included cross-sectional, longitudinal and intervention designs.
Results
To date, there is good emerging evidence that dietary supplementation with fish oil may be of some benefit and this is enhanced if combined with aspirin. All clinical intervention studies to date have been on small sample sizes, and this indicates there is need for larger and more robust clinical trials to verify these initial findings.
Conclusions
Dietary supplementation with fish oil could be a cost-effective adjunctive therapy to the management of periodontal disease.
Clinical relevance
The host modulatory properties of omega-3 fatty acids warrant further assessment of their use as an adjunct in the management of periodontitis.

Non-destructive testing techniques to evaluate the healing efficiency of self-healing concrete at lab-scale
Within the European FP7 project HEALCON, Non-Destructive Testing (NDT) and monitoring techniques are developed and combined to characterize the effects of self-healing mechanismsin small and full-size specimens. In the first stage, healing mechanisms were evaluated at lab-scale. Specimens containing encapsulated polymer precursors were cracked and reloaded after the healing period. During loading, healing and reloading, NDT techniques (acoustic emission analysis, vibration analysis and ultrasonic measurement) were applied to help understanding the cracking behavior, capsule breakage and healing efficiency. Moreover, the effect of the flexibility of the polymeric healing agent on the crack re-opening during reloading was investigated on cracked and healed mortar specimens, using acoustic emission and digital image correlation techniques. The results show the applicability of NDT methods to evaluate the self-healing efficiency for small specimens. Comparing the NDT techniques, some of them (e.g. ultrasound) seem to be good candidates for in situ monitoring of the healing efficiency.

From a biological sample to high-quality digital image representation: avoiding and restoring image artifacts through proper acquisition and image restoration
The objective of modern microscopy is to acquire high-quality image based data sets. A typical microscopy workflow is set up in order to address a specific biological question and involves different steps: sample preparation, image acquisition, image storage, restoration (if necessary) and analysis. In order to analyze the images and draw scientific conclusions, it is crucial to obtain image data that reflects reality as close as possible. We provide an overview of the fundamental artifacts and degradations that affect many micrographs during the acquisition and storage phase: non-uniform illumination, blur, noise, digitization and compression. Image restoration techniques such as flat-field correction, deconvolution and denoising can manipulate the acquired data in an effort to reduce the impact of artifacts due to physical and technical limitations, resulting in a better representation (i.e. a more accurate correspondence to reality) of the object of interest. However, precise usage of these algorithms is necessary in order to not introduce further artifacts that might influence the data-analysis and bias the conclusions. For example, high compression rates may result into small file sizes, but typically introduce blocking artifacts that might affect consecutive image segmentation. It is essential to understand image acquisition and storage, and how it introduces artifacts and degradations in the acquired data, so that their effects on subsequent analysis can be minimized. We describe why artifacts appear, in what sense they impact overall image quality and how to mitigate them by first improving the acquisition parameters and then applying proper image restoration techniques.

Weight loss in children: which diagnosis to think about and how to handle The interpretation of „insufficient weight" and the differential diagnosis of weight loss in children have many differences when compared to the approach for adults. In children, weight stagnation, poor weight gain and weight loss have to be taken seriously since it is often the first symptom of an underlying disease. To interpret a child's weight in a correct manner, it is crucial to use weight and height percentiles. A minimum of technical examinations besides an anamnesis and a clinical examination is necessary. A dietary intake will be needed to evaluate a sufficient intake. A stool examination for parasites, a urine examination, a blood test with antibodies for coeliac disease and a measurement of the faecal calprotectin above the age of five are the first-line technical investigations. This manuscript is a centre-based expertise on how to handle weight loss in children, using case-reports as an example.

Robust sensorless load angle control for stepping motors
In industry, the bulk of the stepping motors is driven in open loop full-step mode with maximum current to avoid step loss. This results in noisy operation due to torque ripples and a poor energy-efficiency. To tackle these problems the current current level at which the stepping motor is driven can be reduced to an optimal level. In this paper, a sensorless load angle controller is proposed and implemented to optimise the drive current level. However, reducing the current level results in a diminished torque margin for load disturbances. In this paper, a countermeasure to enhance the robustness of the sensorless load angle controller against torque disturbances is proposed and assessed trough measurements.

Energy efficiency measurement procedure for gearboxes in their entire operating range
Over the last decade, forced regulations and a growing social awareness with respect to energy efficiency have resulted in a renewed interest in the research for high efficient electrical machines. When an electrical motor is coupled to a machine, in many cases a gearbox or belt transmission is used. Research shows a lack of information on energy efficiency of these components. In comparison to electrical motors and drives, there is very few regulation and if efficiency values can be found in catalogues, there is no regulated test procedure available to validate the data. As a result, the reliability of these efficiency values is unknown and comparison between manufacturers and technologies is impossible. In this paper a test bench is proposed to measure the energy efficiency of a gearbox with an accuracy up to 0.4%. The test bench is used to measure the efficiency of gearboxes in their entire speed and torque range. Contour maps are used to visualize these measurement results. Moreover, a measurement campaign using different gearboxes is carried out to compare the energy efficiency in the manufacturers catalogue and the measured efficiency.

ObjectiveWe provide a survey of recent advances in biomedical image analysis and classification from emergent imaging modalities such as terahertz (THz) pulse imaging (TPI) and dynamic contrast-enhanced magnetic resonance images (DCE-MRIs) and identification of their underlining commonalities.MethodsBoth time and frequency domain signal pre-processing techniques are considered: noise removal, spectral analysis, principal component analysis (PCA) and wavelet transforms. Feature extraction and classification methods based on feature vectors using the above processing techniques are also discussed. These include Mahalanobis distance, support vector machine (SVM) and extreme learning machine (ELM) classifiers.ValidationExamples where the proposed methodologies have been successful in classifying TPIs and DCE-MRIs are discussed.ResultsIdentifying commonalities in the structure of such heterogeneous datasets potentially leads to a unified multi-channel signal processing framework for biomedical image analysis.ConclusionThe proposed complex valued classification methodology enables fusion of entire datasets from a sequence of spatial images taken at different time stamps; this is of interest from the viewpoint of inferring disease proliferation. The approach is also of interest for other emergent multi-channel biomedical imaging modalities and of relevance across the biomedical signal processing community.
