Les anomalies congénitales des branches mandibulaires et de l'articulation temporo-mandibulaire sont prises en charge chirurgicalement depuis le début du XXe siècle. Pour autant, les résultats tant morphologiques que fonctionnels restent souvent décevants, expliquant le recours à des interventions itératives. Il n'existe de nos jours toujours pas de réel consensus concernant le type d'intervention à proposer et l'âge de réalisation. Pour les formes les plus mineures, le recours aux chirurgies « classiques » orthognatiques, voire au protocole de distraction ostéogénique semble donner de bons résultats, surtout s'ils sont réalisés en fin de croissance. Dans les formes sévères, il est souvent nécessaire de procéder en plusieurs étapes chirurgicales, le plus souvent en commençant par une greffe chondrocostale, surtout en cas de chirurgie interceptive, réalisée avant la fin de la croissance pour améliorer la qualité de vie du patient.Congenital deformities of the mandibular ramus and of the temporo-mandibular joint are treated by surgery since the early 20th century. However, morphological and functional results are often disappointing, accounting for iterative operations. Today, a clear consensus concerning the type of intervention to be proposed, and at what age it should be carried out does not yet exist. For mild cases, "conventional" orthognathic or osteogenic distraction procedures seem to work well, especially if they are carried out at the end of growth. In severe cases, it is often necessary to proceed in several surgical steps, usually starting with a chondrocostal graft, especially when interceptive surgery, performed before the end of growth, is preferred in order to improve the patient's quality of life.
