Results of Atypical Hemifacial Spasm with Microvascular Decompression: 14 cases reported and lectures reviewed.

Abstract
OBJECTIVE: There were few studies reporting atypical hemifacial spasm(AHFS) and the mechanism of AHFS is still unclear. The purpose of this article was to study the etiology, prognosis and treatment of atypical hemifacial spasm.
METHODS: We retrospectively analyzed the clinical data for 14 consecutive patients that underwent microvascular decompression(MVD) for AHFS from January 2014 to December 2015. The clinical feature, outcome and complications were observed.
RESULTS: The incidence of atypical hemifacial spasm was 1.52%. There were no significant differences in sex, age, side and duration between the typical HFS and AHFS. During the follow-up period, the effective rate of MVD for patients with AHFS was 92.3%. 3 patients suffered delayed facial palsy(DFP) postoperatively, 1 patient experienced occipital sensory disturbance and 1 case suffered hearing loss.
CONCLUSION: Most cases of AHFS were caused by neurovascular conflict on the zone IV of the facial nerve distal to the root entry zone. Microvascular decompression surgery was the first choice for atypical hemifacial spasm.
